Histology of Epithelial Skin Tumors

Although this topic is still a matter of debate, actinic keratoses can now be considered to\r\nrepresent early squamous cell carcinomas in situ.1,10,24,41 However, it appears that only\r\na small proportion of actinic keratoses will develop into an invasive squamous cell carcinoma.\r\nRegression of some cases of actinic keratoses has also been reported, most likely as a\r\nresult of immune mechanisms.25 Metastases after transformation of actinic keratosis into invasive\r\nsquamous cell carcinoma are very rare except for those tumors that arise on the ear, lip,\r\nanus and vulva, which have been reported to be often associated with a more aggressive\r\nbehaviour.26 Actinic keratoses are characterized by dysplasia of varying degree (from mild changes\r\nthrough to typical carcinoma in situ), predominantly in keratinocytes of the interadnexal epidermis\r\n(Fig. 1). Parakeratosis is usually present. Variants of actinic keratosis include the hyperplastic,\r\nthe proliferating, the atrophic, the acantholytic, the epidermolytic and the bowenoid\r\nsubtype. The dermis underlying and adjacent to actinic keratoses typically shows the presence\r\nof ectatic vessels and solar elastosis.
